After you first contact the office to schedule an appointment, you will receive an email with a link to our client portal. You will then be able to access, review, and sign the necessary forms for treatment and fill out an intake questionnaire about your history and presenting concerns. You will also be asked to enter your insurance and credit card information.
If you do not have access to email and would prefer fill out the necessary forms by hand, you will find the forms below: Intake Assessment, Consent to Treat and Insurance Authorization Form, Treatment & Financial Policies, and Notice of Privacy Practice. Please bring the completed forms, your insurance card and photo ID to your first therapy session.
